"Form 1099-R" is a form issued by the Internal Revenue Service, used to report individual distributions from profit sharing plans, IRAs, pensions, insurance contracts, and annuities. With this form, the tax filer reports the gross distribution of funds that are taxable paid during that tax year as well as any federal income tax that was withheld.
