Questions Remain in Death of Middletown, Ohio, Steel Mill Worker.

Aug. 20--MIDDLETOWN, Ohio--A U.S. Occupational Safety and Health Administration probe into the on-the-job death of an AK Steel worker last month has pointed to at least one area of concern: Before the accident, the operator of the overhead crane could not see how close the lower crane block was getting to the crane's upper block.
