affidavit made by the executor of an estate that certifies the decedent's place of residence at the time of death. Before securities can be transferred from an estate, it must be verified that no liens exist against them in the home state of the decedent.
notarized form stating the legal residence of a deceased person. It may be executed by an individual familiar with the facts, such as an executor, survivor, or attorney. The form is often required when a shareholder dies and was residing in a state that is different from the address on the account. Also known as an affidavit of residence.
