OSHA cites steel erector in collapse at Chicago post office. (Occupational Safety and Health...

The U.S. Occupational Safety and Health Administration has issued citations to a structural steel fabricator and erector in connection with a collapse last November that killed two iron-workers and injured five at a 1.5 million-sq.-ft. U.S. postal facility under construction in Chicago.

The citations, issued to Pitt-Des Moines Inc., Melrose Park, Ill., allege three safety violations and carry proposed penalties totaling $147,000.

The first "willful" violation was the failure to utilize two bolts and nuts at each connection prior to releasing the crane hoist
